Trying to find Supported & Independent Living Services & In-Home Respite Care for adults with developmental disabilities can seem like a scary and intimidating prospect.  We've put together a simple 3 phase road map to help guide you through this process.

Trying to find Supported & Independent Living Services & In-Home Respite Care for adults with developmental disabilities can seem like a scary and intimidating prospect.  We've put together a simple 3 phase road map to help guide you through this process.

Phase 1: Start with your Regional Center

Step 1

The first step is to contact your  local Regional Center to request a service eligibility assessment for you or your loved one.  

Step 2

The Regional Center will assign you a Service Coordinator to work with who will help you identify the appropriate services needed.

Step 3

Your Service Coordinator will help you recommend a service provider like New Heights Ventures.

Phase 2: Meet with New Heights Ventures

Step 4

After choosing New Heights Ventures, your Service Coordinator will send us your referral packet and help schedule an evaluation.

Step 5

You meet with us, we get to know each other, and review the types of services that are offered.  We want to make sure we are a good fit for your needs. 

Step 6

If you are comfortable with us and feel we are a good fit for your needs, we will request authorization from the Regional Center to fund the services that will be provided. 

Phase 3: We begin care

Step 7

After authorization by the Regional Center, New Heights Ventures can start.
